Work Environment

This position is fully remote, allowing for flexible scheduling while ensuring accessibility to patients within our hospice center's service area. Our telehealth platform is user-friendly and compliant with HIPAA regulations, enabling effective virtual communication with clients in the comfort of their homes. Key Responsibilities

• Conduct comprehensive assessments and evaluations of clients to determine their needs and establish effective treatment goals.
• Facilitate individual therapy sessions and group therapy workshops focused on coping strategies, grief processing, and emotional support, utilizing evidence-based practices such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT).
• Collaborate with a multidisciplinary team, including physicians, nurses, and other social workers, to develop and implement individualized treatment plans that address both mental health and social determinants affecting our clients.
• Provide crisis intervention strategies and resources, ensuring timely access to comprehensive support for clients experiencing acute distress.
• Maintain thorough and up-to-date electronic health records in accordance with HIPAA regulations, ensuring the confidentiality and integrity of patient information.
• Participate in regular case reviews and team meetings to discuss progress, share insights, and drive continuous improvement in treatment approaches.
• Engage in ongoing professional development opportunities and training to remain informed about the latest trends and effective methodologies in mental health counseling.

Patient Population / Client Focus

Our primary client base comprises individuals facing end-of-life challenges, including patients in hospice care and their families. The ideal candidate will be sensitive to the unique emotional and psychological issues faced by these clients, demonstrating the ability to provide empathetic support that promotes dignity and solace during difficult times. Your expertise will significantly contribute to enhancing the quality of life for these individuals by addressing their mental health needs.

Qualifications

• A Master’s degree in Social Work from an accredited program.
• Valid Licensed Clinical Social Worker (LCSW) license in the state of Texas.
• A minimum of 2 years of professional experience in mental health counseling, preferably with exposure to hospice or similar care settings.
• A comprehensive understanding of various therapy modalities, particularly evidence-based practices relevant to mental health counseling.
• Proficient in using telehealth technology to facilitate remote therapy sessions effectively.
• Demonstrated ability to work autonomously while thriving in a collaborative team environment.
• Exceptional verbal and written communication skills, with the ability to engage effectively with clients and colleagues alike.

Preferred Experience

• Experience providing mental health counseling in a hospice or palliative care setting.
• Familiarity with treatment programs focusing on grief and loss.
• A background in assessment tools and care coordination.

Compensation and Benefits
