Key Responsibilities\n

• Utilize your social work education, training, and experience to independently assess and formulate psychosocial diagnoses for clients, ensuring that thorough evaluations inform treatment decisions.

• Collaborate with clients to develop individualized treatment plans that center on their unique needs, preferences, and goals for recovery.

• Implement evidence-based behavioral interventions such as solution-focused therapy, motivational interviewing, and problem-solving treatment, adapting your approach to fit each client’s circumstances.

• Coordinate care by working alongside a team of clinicians, including psychologists and primary care providers, ensuring an integrated approach that addresses all aspects of the client’s health.

• Monitor and track patients' progress toward their treatment goals, using both in-person and virtual check-ins to assess treatment effectiveness and make necessary adjustments.

• Support medication management as prescribed, focusing on adherence to psychotropic medications, discussing potential side effects with clients, and evaluating overall treatment effectiveness.

This role primarily involves working with individuals experiencing substance use disorders who are in need of detoxification and subsequent mental health support. Qualifications\n

• A Master's degree in Social Work from an accredited program.
