Our outpatient office serves as a hub for clinical services, ensuring that our clients receive ongoing support and care coordination.

Key Responsibilities\n

• Conduct comprehensive mental health assessments using clinical interviews and standardized tools to identify patient needs and appropriate interventions.

• Utilize evidence-based therapeutic approaches to provide individual, group, and family therapy that addresses a variety of mental health and behavioral issues.

• Develop and implement personalized care plans tailored to each patient, setting measurable goals and objectives to track progress and therapeutic outcomes.

• Coordinate care by establishing referrals to community resources and ensuring a continuity of care through effective follow-up procedures.

• Maintain precise and up-to-date documentation of assessments, treatment plans, progress notes, and discharge summaries, in adherence to regulatory and organizational guidelines.

• Engage in regular consultations with other clinical staff and providers to enhance interdisciplinary team collaboration and patient care.

Patient Population / Client Focus\n

You will work with a diverse client population, including individuals grappling with anxiety, depression, substance abuse, and other mental health concerns. Qualifications\n

• A minimum of 3 years of clinical experience in mental health counseling, focusing on recent and relevant practice.

• A Master's Degree in Counseling, Social Work, Marriage & Family Therapy, or Psychology from an accredited institution is mandatory.

• Must possess a current State of Pennsylvania license as an LCSW, LMFT, or LPC, allowing you to practice within the state.

• Experience with electronic health records (EHR) and documentation standards will be beneficial.

Compensation and Benefits\n
