Key Responsibilities\n

• Conduct comprehensive assessments to identify the needs and concerns of patients, ensuring tailored therapeutic interventions.

• Collaborate effectively with a multidisciplinary team, including supervisors, direct care staff, and external family members, to communicate patient progress and adjust treatment plans as necessary.

• Develop a structured schedule for patient appointments, in-home visits, and regular monitoring of plan implementation.
\n
• Lead and coordinate home visits in compliance with care requirements, guiding observations, chart reviews, and any necessary data collection.
\n
• Ensure that all components of the program plan are executed efficiently and deliver the desired outcomes.
\n
• Participate in Interdisciplinary Team Meetings and contribute to strategic discussions regarding patient care and program efficacy.
\n
• Compile and maintain detailed monthly and quarterly progress notes to document patient advancements and ongoing needs.
\n
• Facilitate training sessions for staff to ensure they are equipped with the skills and knowledge necessary to implement the program plans and documentation standards.
\n
• Perform follow-up interviews and assessments, ensuring ongoing support aligns with patient needs and treatment goals.
\n
• Serve as a liaison between our organization and the broader mental health community, fostering relationships that support the development of accessible mental health resources.

• Contribute to the creation of training materials and procedural documentation aimed at enhancing the services provided to individuals.

Patient Population / Client Focus\n

As an LCPC at Motion Rehab Group, you will have the opportunity to work with diverse populations including both adults and youth. Your expertise will address a range of mental health concerns, such as anxiety, depression, trauma, substance misuse, and family dynamics. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.

• A minimum of two years’ experience in a counseling setting, preferably in a clinical context where you have developed strong therapeutic skills and understanding of mental health needs.

• Strong grounding in ethical and legal standards relating to mental health practices.
\n
• Exceptional interpersonal and communication skills, with the ability to build rapport and trust with a diverse clientele.

• Demonstrated ability to collaborate with a team of healthcare professionals to optimize patient outcomes.

• Familiarity and proficiency with Electronic Medical Records (EMR) software and related documentation technologies.
