Job Title: Physical Therapist: Home Health Location: Stroudsburg, PA Setting: Home Health Pay Range: 2407.20 - 2527.56 Schedule: 07:00 - 15:00 (D/N) Contract Length: 13 weeks Candidate Type: Traveler Requirements: Years years of experience as a PT; Qualified applicants MUST have at least YEARS years of experience in Home Health; Valid Physical Therapist license; Be willing to obtain STATE licensure Responsibilities:

• Deliver one-on-one, high-touch care to patients in their homes.
• Conduct skilled nursing visits and perform assessments.
• Create and manage individualized care plans.
