Key Responsibilities\n

The LCPC will engage in the following responsibilities to ensure an effective therapeutic process:

• Conduct thorough assessments to identify the needs and preferences of patients, ensuring that treatment plans are tailored to individual circumstances.

• Maintain open lines of communication with supervisors, direct care staff, family members, and other stakeholders to discuss patient progress and adjust treatment plans appropriately.

• Prepare and share schedules for appointments, home visits, and patient observations with supervisory staff to ensure continuity of care.
\n
• Conduct home visits to gather firsthand observations and insights into the patient's living situation and support systems.
\n
• Implement and monitor treatment plans effectively, ensuring that interventions are carried out as designed.
\n
• Participate in Interdisciplinary Team Meetings and contribute to case discussions to enhance comprehensive patient care.
\n
• Document monthly and quarterly progress notes detailing patient developments and treatment efficacy.
\n
• Organize and lead training sessions to enhance team members' skills in program implementation and documentation practices.
\n
• Provide ongoing support through follow-up interviews and home visits to assess the progress of treatment goals.
\n
• Act as a liaison with community mental health resources, assisting with the development of support networks for patients.
\n
• Assist in the creation of training materials to improve services and outcomes for individuals served.

Patient Population / Client Focus\n

Our counselors work with a diverse range of clients, including adults and youth from various backgrounds and with varying mental health challenges. You will have the opportunity to specialize in areas such as family therapy, couples therapy, substance misuse, or trauma recovery, providing you with a unique platform to impact patients' lives positively.

Qualifications\n

To be successful in this role, candidates must meet the following qualifications:

• Hold a Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Possess an active and unrestricted licensure as a Licensed Clinical Professional Counselor (LCPC) in Illinois.

• Have a minimum of two years of experience in counseling, particularly within clinical settings.

• Demontrate a strong knowledge of ethical and legal standards in mental health practice.
\n
• Exhibit excellent communication and interpersonal skills, essential for building rapport with a diverse client base.

• Show proficiency with Electronic Medical Records (EMR) software and technology-related tools.

Preferred Experience\n

While not required, candidates with specialized experience or training in family therapy, trauma-informed care, or substance use treatment will be prioritized.

Compensation and Benefits\n
