- Per-diem flexibility — build visits around your availability
- Serve patients in Palm Beach County
- Work with an experienced home health clinical and operations team
- Opportunity for additional visits as our census continues to grow
- Make a direct impact by helping patients regain strength, mobility, safety and independence at home
- Current Florida Physical Therapist license
- Medicare-certified Home Health experience
- Experience completing home health evaluations, reassessments and discharges
- Strong clinical documentation and care coordination skills
- Reliable transportation and ability to travel within the assigned territory
- Commitment to timely, patient-centered care
Territory ???? Palm Beach County, Florida
We are especially interested in PTs who already work in home health and are looking to add quality per-diem visits to their existing schedule.
