Key Responsibilities\n

• Conduct comprehensive assessments, utilizing evidence-based approaches to determine the unique mental health needs of each patient.

• Maintain continuous communication with supervisors and other team members, ensuring aligned care strategies that facilitate optimal patient outcomes.

• Prepare and share a tentative schedule outlining appointments and follow-up activities, such as home visits or virtual check-ins, tailored to each client’s needs.

• Coordinate home visits and ensure comprehensive evaluations to meet the unique requirements of clients requiring in-home support.

• Implement and monitor therapeutic interventions, adapting strategies as necessary to achieve effectiveness based on ongoing evaluations.

• Participate proactively in Interdisciplinary Team Meetings, contributing your insights to enhance program development and patient care strategies.

• Document comprehensive monthly and quarterly progress notes reflecting client developments, treatment adherence, and necessary adjustments.

• Organize and deliver training sessions to help ensure staff members are proficient and up-to-date on implementation protocols and documentation requirements.

• Provide consistent follow-up checks and reconnect with clients through interviews or home visits for continued support.
\n
• Actively maintain comprehensive records, ensuring easy accessibility to information for relevant stakeholders.

• Serve as a liaison between the mental health community and the public, advocating for mental health awareness and development goals.

Patient Population / Client Focus\n

The LCPC will work with a diverse array of clients, including children, adolescents, and adults facing a variety of challenges, from anxiety and depression to trauma and substance misuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential.

• Possession of an active and unrestricted Clinical Professional Counselor license in the state of Maryland is required.

• Experience in specialized areas such as family therapy, couples therapy, trauma-informed care, or substance abuse counseling is highly preferred.
