Key Responsibilities\n

In this role, you will report to the Social Work Manager and your core responsibilities will include:

• Developing and implementing individualized treatment plans based on evidence-based methodologies, which may include both individual and group therapy sessions.

• Providing education and support to patients, families, and school staff regarding mental health issues, treatment options, and needed community resources to foster a supportive network.

• Facilitating comprehensive care coordination by consulting with medical professionals and educators to enhance compliance and improve overall patient experiences within the school environment.

• Maintaining detailed and accurate documentation of patient interactions and progress in accordance with departmental policies and standards.
