Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions and capabilities through various diagnostic and prognostic assessments, including muscle and joint function tests.
• Develop and implement personalized treatment plans that include therapeutic exercises and modalities like traction and massage to relieve pain and improve function.
• Lead, oversee, and mentor physical therapy assistants to ensure optimal delivery of care and adherence to prescribed treatment plans.
• Regularly assess patients' progress, adjusting care plans as necessary and documenting updates in clinical records including initial evaluations, progress notes, and discharge summaries.
• Engage with both patients and their families to instruct them in the effective use of rehabilitation devices, ensuring a thorough understanding of care protocols.
• Collaborate in interdisciplinary team meetings, contributing to case discussions and treatment planning to achieve comprehensive patient care.
• Conduct home environment evaluations to recommend adjustments that foster patient independence and facilitate recovery.
• Observe and report any changes in a patient's condition to the physician and case manager promptly, ensuring that the care plan is responsive to the patient's evolving needs.

Qualifications

• A bachelor's degree in Physical Therapy from an accredited institution.
• A valid Physical Therapy licensure in New Jersey, with a commitment to maintaining good standing.
• Current CPR certification and evidence of ongoing professional development.
• A valid driver's license and automobile insurance to facilitate travel to patient homes if required.

Preferred Experience

• At least 1 year of experience working as a physical therapist, ideally in an acute care or home care setting.
• Evidence of creative thinking and problem-solving abilities, especially in challenging clinical scenarios.
• A demonstrated commitment to clinical excellence and a passion for improving patient outcomes.

Compensation and Benefits
