Key Responsibilities\n

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive assessments to better understand each client's unique circumstances and challenges.

• Creating personalized and detailed treatment plans in collaboration with clients, ensuring measurable and achievable goals are set.

• Connecting clients with necessary resources, such as financial assistance, housing support, and community services to alleviate barriers to care.

• Providing crisis intervention for clients experiencing acute mental health crises, including suicidal ideation, severe trauma responses, or substance abuse issues.

• Engaging in educational outreach initiatives to promote mental health awareness and reduce stigma within the community.

• Maintaining meticulous and up-to-date records, treatment plans, and progress notes, adhering to HIPAA guidelines and all relevant regulations.

• Receiving and giving ongoing mentorship to promote both personal and professional development.

Patient Population / Client Focus\n

Your primary focus will be working with individuals and families utilizing hospice care. This often entails supporting those facing terminal illnesses, providing emotional and psychological guidance, as well as addressing the needs of their caregivers. You will utilize your LCSW expertise to foster resilience and help clients cope with challenging life circumstances in a sensitive and empathetic manner.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A valid License to Practice as a Clinical Social Worker (LCSW) in the state of Virginia, with capacity to operate independently.

• Established experience (minimum of 2 years) in providing therapeutic services to individuals, couples, and families.
