Key Responsibilities

• Conduct comprehensive evaluations of new patients, including reviewing their medical history, performing standardized assessments, and developing individualized treatment plans based on their specific needs.
• Communicate evaluation results and treatment strategies effectively to patients and their families, ensuring a shared understanding of therapeutic goals and interventions.
• Implement treatment protocols with precision based on in-depth evaluations, patient responses, and best practices in contemporary rehabilitation.
• Engage in interdisciplinary consultations with other healthcare professionals when necessary, documenting changes in conditions that may require adjustments to the initial care plan.
• Ensure appropriate discharge planning and follow-up care, documenting all patient interactions clearly and thoroughly to maintain accurate medical records.
• Demonstrate effective time management skills by maintaining a structured daily schedule to meet productivity goals while providing the best possible care to each patient.
• Participate in community outreach efforts and marketing initiatives aimed at raising awareness of available rehabilitation services.
• Continuously seek opportunities for professional development, remaining abreast of advances in physical therapy practices and acting as a clinical mentor for student interns.

Patient Population / Client Focus

Your primary focus will be on providing physical therapy services to a diverse range of patients in their homes, including individuals recovering from surgery, those managing chronic conditions, and patients seeking rehabilitation following traumatic injuries. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• Current and valid state license to practice as a Physical Therapist.
• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.

Compensation and Benefits
