Located in the vibrant city of Tampa, our home care services allow us to provide individualized care in a comfortable setting for our patients. This position requires a commitment to high-quality patient interactions within a collaborative team environment comprising two physical therapists and one physical therapist assistant (PTA). You will play a crucial role in enhancing patient recovery, informing direct engagement strategies, and upholding the standard of care we promise our clients.

Assess patients to identify their specific needs and rehabilitation goals, developing tailored care plans.

Monitor and document the progress of 2-3 patients per hour, making adjustments to treatment plans as required.

Provide education to patients and families about the rehabilitation process and self-management strategies to maintain optimal health.

Collaborate with fellow medical professionals to ensure comprehensive care coordination, sharing insights from treatment analyses.

Utilize electronic medical record (EMR) software, such as WebPT, to maintain accurate patient documentation and treatment histories.

Constantly evaluate treatment outcomes, applying evidence-based practices to enhance patient results and the overall effectiveness of our therapy programs.

Our patient population primarily includes individuals recovering from orthopedic injuries, surgeries, and chronic pain conditions. As a key member of our team, you will work with a diverse clientele, focusing on those who need personalized rehab plans that align with their lifestyle and functional goals. Your efforts will directly contribute to reducing reliance on prescription medications and promote a non-surgical approach in managing pain and mobility issues.

Doctor of Physical Therapy (DPT) degree with a focus on outpatient orthopedic care.

Current and valid Physical Therapist License for the state of Florida.

Strong manual therapy skills coupled with knowledge of therapeutic modalities.

Proficiency in utilizing EMR software solutions like WebPT for comprehensive patient management.

Previous experience in a home care setting, allowing for familiarity with the unique demands and expectations of providing care in a patient's home.

Background in sports medicine or working with an athletic population would be highly advantageous.

Experience in conducting workshops or group therapy sessions to foster community engagement in physical health.
