Work Environment

This role involves working onsite at patients' homes across Tampa, Florida. As a physical therapist in home care, you will experience diverse work settings that vary with each patient's needs. Key Responsibilities

• Plan, implement, and supervise comprehensive physical therapy treatment programs to improve patients' mobility, strength, and overall function based on individual care plans.
• Conduct thorough assessments of patients' physical abilities by employing diagnostic and prognostic tests to evaluate muscle and joint function.
• Oversee and guide physical therapy assistants, ensuring they effectively deliver services and adhere to established care plans.
• Improve the patient's home environment for better rehabilitation outcomes, making recommendations as necessary to facilitate independence.
• Administer manual therapeutic exercises and instruct patients on non-manual exercises to enhance their treatment progress.
• Document patient evaluations, treatment progress, and updates on case management, including OASIS assessments and discharge summaries.
• Collaborate with interdisciplinary teams to participate in care conferences, ensuring comprehensive treatment strategies are aligned with patient goals.
• Educate patients and their families about treatment protocols, including the use of therapeutic devices and guidance on ongoing care.
• Maintain accurate and timely documentation, ensuring compliance with branch policies and state regulations.
• Complete functional assessments on-site, revising care plans based on patient progress and evolving needs.

Patient Population / Client Focus

The successful candidate will primarily work with a diverse patient population, including individuals recovering from surgeries, those managing chronic conditions, and elderly clients in need of rehabilitation support. Qualifications
