Key Responsibilities\n

• Communicate effectively with physicians and other healthcare providers to relay critical updates on patient progress, concerns, and treatment needs.

• Collaborate with healthcare team members in developing comprehensive treatment plans and discharge strategies, drawing from psychosocial assessments and established intervention methods.

• Assist clients with discharge planning and follow-up care coordination, ensuring smooth transitions to appropriate services.

Compensation and Benefits\n
