Work Environment\n

This full-time position offers flexibility through a remote work setting, allowing you to provide care from the comfort of your home while maintaining an essential connection to the Hospice Center. You will have access to a climate-controlled environment and will regularly move around the facility, engaging with other healthcare professionals to ensure comprehensive and coordinated patient care.

Key Responsibilities\n

• Conduct thorough assessments and evaluations of patient mental health status, employing tools such as the Diagnostic and Statistical Manual of Mental Disorders to inform diagnosis and treatment planning.

• Create individualized care plans that reflect the cultural diversity and specific needs of the patient population we serve, ensuring that all care approaches are personalized and relevant.

• Implement evidence-based therapeutic interventions to effectively support the mental health diagnosis, regularly monitoring and adjusting treatment strategies as necessary based on patient progress.

• Collaborate with patients, families, and interdisciplinary teams to formulate comprehensive treatment strategies aimed at achieving desired health outcomes.
\n
• Maintain accurate and timely documentation of patient treatment records and other clinical information in accordance with regulatory standards and departmental protocols.

• Facilitate referrals to additional treatment options and coordinate care, effectively bridging gaps between patients and external healthcare providers.

• Provide mentorship and educational opportunities to students, interns, residents, and volunteers, fostering the next generation of mental health practitioners.

• Participate in clinical review processes under your supervisor’s guidance, including peer audits, chart reviews, and collaborative case discussions, to enhance clinical practices across the team.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

Compensation and Benefits\n

The salary for this position ranges from USD 90,000 to USD 105,000 annually, commensurate with experience and qualifications.
