Work Environment

This position is based onsite, where you will interact directly with clients in their living environments, allowing for a personalized assessment and treatment strategy. You will work closely with an interdisciplinary team dedicated to holistic and effective patient care. Key Responsibilities

• Direct physical therapy treatment to help prevent disability, alleviate pain, enhance muscle function, and maintain maximum performance based on the patient's capabilities and care plans.
• Oversee and collaborate with physical therapy assistants to ensure seamless implementation of individualized treatment plans.
• Conduct thorough evaluations of patients' functional abilities through diagnostic tests involving muscles, nerves, joints, and overall movement.
• Assess the home environment to identify modifications needed for enhanced independence and make necessary recommendations.
• Administer therapeutic exercises, both manual and non-manual, ensuring proper guidance and motivation for clients as they engage in their rehabilitation exercises.
• Implement traction, massage, and other therapeutic treatments utilizing physical agents to facilitate recovery and rehabilitation.
• Evaluate, adjust, and fit prosthetic and orthotic devices, providing recommendations for modifications when necessary.
• Participate actively in developing and updating patient care plans through collaboration with the healthcare team.
• Monitor and document any changes in the patient's condition, adjusting treatment plans and communicating findings with physicians and case managers.
• Maintain comprehensive clinical records, including initial evaluations, care plans, and progress documentation, ensuring they adhere to regulatory standards.
• Engage in case conferences and interdisciplinary meetings to facilitate coordinated and comprehensive treatment approaches.
• Provide education and support to clients and their families regarding their rehabilitation program and the use of necessary equipment.
• Complete requisite documentation efficiently while observing all healthcare policies and state regulations.
• Perform on-site functional assessments, utilizing community resources to enhance patient support and rehabilitation outcomes.

Patient Population / Client Focus

Our patient population primarily includes individuals recovering from orthopedic surgeries, injuries, or chronic musculoskeletal conditions. Our focus is on adult and geriatric clients, many of whom require tailored intervention strategies to regain independence and enhance their quality of life. By employing a personalized and compassionate approach, our therapists effectively address the unique challenges faced by each patient in their own home environment.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A minimum of 1 year of experience in a physical therapy setting, preferably with a focus on orthopedics.
• Current licensure as a Physical Therapist in the state of Florida.
• Strong communication and interpersonal skills to collaborate with team members and interact positively with patients and families.
• Proficiency in patient assessment and evaluation related to orthopedic conditions.

Preferred Experience

While not mandatory, candidates with additional certification in orthopedic rehabilitation or previous home healthcare experience will be viewed favorably. Familiarity with documentation requirements, OASIS assessments, and the ability to work autonomously in a home setting is advantageous.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
