Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans that address the unique needs of each patient.

• Facilitate ongoing communication between caregivers, educators, family members, and the treatment team to monitor progress and adjust care as necessary.

• Schedule and conduct therapy sessions, home visits, and interdisciplinary team meetings to ensure collaborative support for clients.

• Provide training to staff and team members on the implementation of treatment plans and documentation standards to maintain program integrity.

• Document patient progress through monthly and quarterly notes, ensuring adherence to legal and ethical standards.

• Actively engage with the broader mental health community to advocate for clients and contribute to community mental health initiatives.

• Participate in chart reviews and data analysis to enhance program effectiveness and client outcomes.

Patient Population / Client Focus\n

The Licensed Professional Counselor will work with a diverse range of clients, including both adults and youth, reflecting various backgrounds and psychological needs. Clients may be facing challenges such as anxiety, depression, trauma, or family dynamics, requiring a tailored, compassionate approach that prioritizes their unique experiences.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is required.

Compensation and Benefits\n
