Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate emotional, social, and environmental factors impacting patients and their families.

• Develop and implement individualized care plans utilizing appropriate social work interventions tailored to each patient's unique circumstances, health status, and prognosis.

• Coordinate with the healthcare team to involve patients and their families in care planning and decision-making processes.

• Facilitate comprehensive discharge planning by liaising with consulting physicians, insurance providers, and other healthcare entities to ensure seamless transitions of care.

• Deliver education and counseling to patients and families regarding coping strategies, resource navigation, and adaptation to illness and life transitions.

• Provide crisis intervention and therapeutic support, offering both short-term and long-term solutions for emotional distress as needed.

• Share your expertise with colleagues through training sessions and presentations on psychosocial aspects of oncology and hospice care.

• Engage with the community through educational initiatives designed to enhance awareness of resources available for cancer survivors.

• Participate in professional development activities, including research and student training opportunities in the field of oncology social work.

• Provide on-call support to address after-hours inquiries and urgent patient needs.

Patient Population / Client Focus\n

You will primarily serve patients receiving hospice care and oncology services, focusing on individuals with life-limiting illnesses and their families. Your work will involve addressing complex end-of-life issues, emotional distress, and the need for psychosocial support during critical transitions.

Qualifications\n

• Master's Degree in Social Work from an accredited program.

• Current licensure as a Clinical Social Worker (LCSW) in the state of Florida.

• Experience in healthcare, particularly in hospice and palliative care settings, is preferred.

Compensation and Benefits\n

This position offers a competitive salary range of USD 70,000 – 100,000 annually, commensurate with experience and qualifications.
