Work Environment

This role is performed onsite at patients' homes, allowing you to work directly in the environment that most affects their daily lives. As a home care Physical Therapist, you will encounter diverse patient backgrounds and medical conditions, requiring adaptability and innovative problem-solving. Key Responsibilities

• Conduct comprehensive patient evaluations under physician orders and develop personalized treatment plans based on individual patient needs and goals.
• Implement treatment plans through direct therapy, education, and supervision of physical therapy staff and assistants.
• Maintain accurate and timely documentation of patient progress and treatment outcomes, ensuring adherence to the Rehabilitation department's guidelines.
• Provide effective communication of clinical findings in interdisciplinary meetings and collaborate with other healthcare professionals to optimize patient care.
• Participate in patient and family meetings, addressing questions and concerns while educating families about the rehabilitation process.
• Supervise Physical Therapy Assistants and rehabilitation aides as needed, ensuring compliance with state laws and practice standards.
• Engage in ongoing professional development opportunities and educational programs to enhance your skills and stay current in the field.
• Ensure the confidentiality of patient information and adhere to billing procedures for therapy services.

Patient Population / Client Focus

In this position, you will work with a diverse range of patients from various demographics, including seniors, individuals recovering from surgery, and those with chronic conditions. You will help facilitate their recovery process and improve their daily functioning by providing targeted physical therapy interventions tailored to their unique situations.

Qualifications

• Valid Florida State License as a Physical Therapist.
• Current Florida Driver's License, registration, and proof of auto insurance.
• Certification in CPR as well as up-to-date physical examination and PPD tests.
• A Bachelor's degree from an accredited Physical Therapy program.
• Strong organizational skills and exceptional verbal and written communication abilities.

Preferred Experience

• Previous experience in home health or outpatient physical therapy settings.
• Familiarity with electronic health records (EHR) and documentation software, facilitating efficient patient management and billing.
• A proactive approach to patient care and interdisciplinary collaboration.

Compensation and Benefits

This position offers a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and qualifications. Additionally, we provide a comprehensive benefits package, including:
