Work Environment

This position is predominantly onsite, as you will be providing services in the homes of patients across the Fords area. Key Responsibilities

• Conduct thorough evaluations of patients recovering from various injuries, surgeries, or illnesses within their home environments.
• Create individualized treatment plans incorporating various therapeutic techniques, including exercise, hands-on therapy, and the use of specialized equipment to alleviate pain and enhance mobility.
• Continuously monitor patient progress by tracking treatment responses and adjusting therapy plans as necessary to align with therapeutic goals.
• Educate patients, families, and caregivers on effective therapy modalities, assistive devices, and self-care practices to support recovery.
• Maintain comprehensive documentation of patient treatments using EMR software such as Web PT, ensuring accuracy and compliance with healthcare standards.
• Engage in continuous professional development through educational workshops, networking, and participation in professional societies relevant to physical therapy.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required for this role.
• Must possess and maintain an active Physical Therapist license in New Jersey, demonstrating a clean record without any sanctions or board proceedings.

• Experience with EMR systems, particularly Web PT, for accurate documentation and patient management.
• Solid understanding of current research-based physical therapy practices, ensuring that treatments are evidence-based and up-to-date.

Preferred Experience

• Preference will be given to candidates with experience in home care settings, demonstrating adaptability and excellent communication skills with patients and their families.
• Prior experience working with a variety of populations across different age groups, specifically in rehabilitation and mobility enhancement.
