Work Environment

This position is strictly onsite, with the physical therapist providing care directly within the clients' residences. Working closely with a team of healthcare professionals, you will navigate various home environments, adapting therapeutic interventions to best fit each client's unique circumstances. Your role demands flexibility and creativity, as every home presents distinct challenges and opportunities for therapy. This is a full-time role, encompassing a Monday to Friday work schedule, allowing for consistent interaction with clients and their families.

Key Responsibilities

• Develop and implement individualized treatment plans aimed at preventing disability, relieving pain, and restoring optimal muscle function according to each patient's needs.
• Supervise and mentor physical therapy assistants to ensure accurate application of the treatment plan.
• Conduct thorough evaluations of clients' functional abilities using diagnostic tests related to muscle, nerve, and joint performance.
• Assess the home environment for potential rehabilitation barriers and provide strategies to enhance client independence.
• Administer a variety of therapeutic techniques, including manual therapy exercises, traction, and functional activity support, as prescribed by a physician.
• Evaluate, fit, and adjust prosthetic and orthotic devices, offering modifications to the care plan as necessary.
• Document all clinical assessments, evaluations, and treatment progress accurately in the client's medical records, including OASIS documentation, care plans, and discharge summaries.
• Collaborate with an interdisciplinary team to assess and plan comprehensive care, regularly participating in case conferences to ensure an integrated treatment approach.
• Maintain open communication with clients and their families, providing education on therapy techniques and assisting with the integration of therapeutic devices.
• Timely submission of clinical and administrative documentation in accordance with company policies, ensuring compliance and efficient operational workflow.
• Provide ongoing support and supervision to Physical Therapy Assistants as required by state regulations, promoting high-quality care delivery.
• Complete functional assessments periodically to adjust treatment plans based on individual client progress and available community resources.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active physical therapist license in the state of New Jersey without any disciplinary history.

• Strong command of electronic medical record (EMR) systems, particularly experience with Web PT for accurate documentation.
• Deep understanding of current physical therapy practices and interventions based on research and evidence-based methods.

Preferred Experience

• Experience working within home healthcare, specifically outpatient therapy, will be considered a strong asset.
• Proven ability to work independently while fostering collaboration amongst healthcare teams.
• Experience with diverse patient populations, including older adults and those with chronic illnesses.

Compensation and Benefits
