Description

• Conduct patient evaluations in either an inpatient or outpatient setting, tailoring specific treatment plans according to individual needs
• Implement physical therapy interventions that emphasize mobility, strength building, balance, and functionality
• Partner with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes
• Provide education to patients and families regarding therapeutic objectives, discharge planning, and home exercise routines
• Maintain comprehensive documentation of evaluations, treatments, and progress in compliance with facility and regulatory standards
• Engage in care team rounds and interdisciplinary discussions to facilitate seamless patient care transitions

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree recognized by an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
