Key Responsibilities\n

• Conduct thorough assessments and develop personalized treatment plans, providing consultation for pre-crisis and crisis scenarios, behavioral guidelines, and medication treatment plans as required.

• Ensure all documentation is completed accurately and in a timely manner, adhering to regulatory standards to maintain compliance.

• Utilize evidence-based screening tools as recommended by the clinical team to accurately assess patient needs and progress.
\n
• Collaborate effectively with interdisciplinary teams in crafting and implementing individualized positive behavior support plans.
\n
• Actively engage in licensing and accreditation procedures, contributing to the clinic’s adherence to industry standards.
\n
• Act as a liaison with various stakeholders, including service systems, state agencies, and community partners, to support effective coordination of care.
\n
• Communicate openly and effectively with team members and external partners to ensure alignment with the clinic’s mission and contractual obligations.
\n
• Cultivate and maintain relationships with local and state agencies, private organizations, and funding sources to enhance the clinic's service offerings.

• Pursue ongoing professional development through attendance at conferences, continued education, and participation in state-wide initiatives.

• Promote the agency’s mission, values, and principles in all aspects of your work while performing any additional duties assigned by the Clinic Director.

Patient Population / Client Focus\n

This position primarily focuses on a diverse client population, including individuals navigating mental health challenges, substance use recovery, and developmental disabilities. You will engage with clients in therapeutic settings, utilizing evidence-based practices to guide your interventions and optimize patient outcomes through recovery-centered care.

Qualifications\n

• A Master’s Degree in a behavioral health field is required for this position.

• Possession of an independent LMHC license issued by the state board is essential for practice.

Compensation and Benefits\n

Grace Hospital offers a competitive annual salary ranging from $86,000 to $103,000, commensurate with experience and qualifications. Additionally, our benefits package includes:
