Key Responsibilities\n

The LPC will be responsible for a variety of tasks critical to the delivery of quality mental health care:

• Collaborate with primary care providers to assess, treat, and manage patients’ mental health and psychosocial needs.

• Conduct comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments followed by tailored treatment recommendations.

• Deliver brief, focused therapeutic interventions to assist patients in navigating their mental health challenges.

• Advise on appropriate referrals to specialty mental health services based on patient assessments.
\n
• Implement and monitor follow-up strategies to evaluate patient progress and adjust treatment plans as needed.

• Develop individualized relapse prevention plans and support patients in maintaining stable functional levels.

• Proactively identify ‘at risk’ patients to enhance preventative care strategies.

• Coordinate with additional treatment providers to ensure a comprehensive approach to behavioral health care.

• Document patient progress meticulously and communicate effectively with primary care providers to inform ongoing treatment plans.

• Empower patients with self-management skills and educational resources necessary for active participation in their recovery journey.

• Provide insights and recommendations to clinic management on behavioral services and areas needing evaluation.

Qualifications\n

To be considered for this position, candidates must meet the following requirements:

• A Master’s Degree in Clinical Counseling or a related field.

• Current LPC (Licensed Professional Counselor) License in Arizona.

• In-depth knowledge of mental health assessments, treatment plan development, and various therapeutic modalities.

• Understanding of developmental psychopathology and effective psycho-therapeutic techniques.

• Strong ability to assess individual patient needs and evaluate provided care.

• Capacity to maintain comprehensive clinical records and prepare detailed reports.

• Proficiency in building collaborative relationships with patients, families, and multi-disciplinary team members.

Preferred Experience\n

• Bi-lingual proficiency in English and Spanish is highly preferred but not mandatory.

• Experience working within a Federally Qualified Health Center (FQHC) or similar community health environments.

Compensation and Benefits\n

This role offers a competitive salary ranging from USD 87,000 to 105,000 annually, based on experience and qualifications.
