Key Responsibilities\n

• Conduct and maintain timely crisis alerts to coordinate with county crisis services for participants effectively.
\n
• Coordinate and thoroughly document transitions from hospital and inpatient services, ensuring smooth care continuity and collaborating with hospital staff for discharge support.
\n
• Utilize crisis de-escalation techniques to support individuals in need, embodying the role of Clinician of the Day when required.
\n
• Perform comprehensive risk assessments and develop safety plans that suit individual needs.
\n
• Complete documentation on the same day for all individuals who may be at elevated risk of crisis service interactions, ensuring adherence to best practices and protocols.
\n
• Report critical incidents within 24 hours in accordance with organizational guidelines.
\n
• Provide after-hours call support on a rotating schedule to maintain consistent care coordination.
\n
• Support proactive measures to avoid crisis service use by working collaboratively with participants.
\n
• Perform additional tasks as directed to enhance program effectiveness and client support.

This role primarily serves adults and adolescents facing various mental health challenges, including anxiety, depression, substance use, and trauma-related disorders. Qualifications\n

• Must possess an independent license (LCSW, LMFT, or LPC) in the state of Oregon or current residence.

• Ability to effectively maintain calm and empathic communication in high-stress situations.

Compensation and Benefits\n
