Key Responsibilities\n

• Conduct comprehensive clinical assessments utilizing evidence-based practices, developing personalized treatment plans aimed at improving mental health outcomes for clients within the hospice care framework.

• Implement various therapeutic modalities, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), adapting interventions to address the unique concerns of patients coping with terminal illnesses and associated mental health issues.

• Maintain meticulous clinical records in compliance with HIPAA and other relevant regulations, ensuring patient confidentiality and data security in all interactions.

• Collaborate with a multidisciplinary treatment team composed of psychiatrists, psychologists, social workers, and other healthcare professionals, offering whole-person care for patients facing complex challenges.

• Participate in case consultations and peer review meetings to continuously improve the quality of mental health care provided, sharing insights and strategies with colleagues.

• Educate patients and their families about mental health conditions, treatment options, and community resources to empower them to manage their circumstances and improve resilience.

• Stay informed about the latest research and trends in mental health, incorporating new knowledge into your therapeutic practice for the benefit of the clients.

Patient Population / Client Focus\n

In this role, you will work primarily with patients in hospice care who may be experiencing anxiety, depression, grief, and trauma as they navigate their end-of-life journey. Your focus will be on enhancing their emotional well-being, helping them to cope with their symptoms, and providing support to their families throughout the process.

Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Active licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) in New York State.

• Experience utilizing solution-focused psychotherapy techniques tailored to address diverse patient needs, particularly within the context of hospice care.

• Strong communication and interpersonal skills to establish rapport and build trust with patients and their families.

• Critical thinking and problem-solving capabilities to resolve challenges that may arise in a complex clinical environment.

Preferred Experience\n

Ideal candidates will have a minimum of three years of relevant clinical experience in mental health counseling, particularly in settings involving end-of-life care or hospice services. Familiarity with industry standards and best practices in hospice mental health will be an asset.

Compensation and Benefits\n
