What You'll Do

• Provide skilled physical therapy services to diverse patients in a home health setting
• Develop and implement customized treatment plans based on patient needs
• Monitor patient progress and adjust treatments accordingly
• Educate patients and caregivers on exercises, mobility, and safety
• Administer and manage physical therapy equipment
• Document all patient care activities accurately in EMR (Meditech Home Care)
• Travel within a 50-mile radius to serve multiple locations
• Maintain compliance with all licensing, certification, and safety standards

What You Need

• Valid state license in Louisiana
• BLS certification
• Minimum 1 year of acute care experience
• General medical/surgical skills
• Valid driver's license

• Experience with IV therapy, wound care, and complex patient conditions (heart failure, diabetes)
• ER or ICU experience (not required but a plus)
