Key Responsibilities\n

• Conduct comprehensive mental health assessments to evaluate patients' emotional and psychological needs, considering their medical history and conditions.

• Develop, implement, and adjust integrated treatment plans that address both psychiatric and medical issues, utilizing evidence-based therapies.

• Provide individual and group therapy sessions tailored to the unique needs and experiences of each patient, employing therapeutic modalities that promote positive outcomes.

• Collaborate with medical providers to ensure coordinated care, enhancing treatment effectiveness and patient satisfaction.

• Offer crisis intervention and support during mental health emergencies, demonstrating clinical judgment and empathy.

• Educate and empower patients by connecting mental health with physical health, fostering self-management skills.

• Maintain accurate, confidential documentation of patient interactions and treatment progress in compliance with legal and ethical guidelines.

• Participate actively in team meetings, case discussions, and interdisciplinary care planning to enhance service delivery.

• Stay informed on the latest research, best practices, and innovations in integrated care to continuously improve service quality.

Patient Population / Client Focus\n

In this role, you will primarily serve adults with co-occurring mental health and medical conditions, including chronic pain management and substance use challenges. Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LICSW, LMHC, or LMFT in the state of Washington.

• Proven ability to address the mental health needs of individuals with complex co-occurring medical and psychological conditions.

• Experience in delivering integrated care within a multidisciplinary team setting.

• Strong communication and interpersonal skills to foster effective patient relationships and collaborate with healthcare professionals.

• Ability to uphold professional boundaries, ethical guidelines, and patient confidentiality.

• Familiarity with electronic health record (EHR) systems and related software is preferred.

Preferred Experience\n

• Experience working in a community health clinic or similar integrated care environment.

Compensation and Benefits\n
