Work Environment

The NP will work onsite in a school environment, collaborating with educational staff, counselors, and the healthcare network. Key Responsibilities

• Conduct thorough assessments of students, evaluating both chronic and acute health conditions, diagnosing, and formulating effective treatment plans tailored to the individual needs.
• Order, interpret, and analyze lab tests and other diagnostic procedures to inform treatment decisions and adjustments as necessary.
• Maintain accurate and compliant documentation using electronic medical record (EMR) systems, ensuring that patient information is securely stored and readily accessible.
• Collaborate closely with local and regional operational and clinical leadership, ensuring alignment of objectives and strategic goals within the community.
• Participate in family conferences, providing essential health education and advanced care planning tailored to each family’s wishes.
• Actively engage in interdisciplinary care teamwork, coordinating with primary care providers, specialty services, and telemedicine resources as needed.
• Fulfill all regulatory requirements, including conducting assessments for mandated monthly, quarterly, and annual visits.
• Respond effectively to pharmacy recommendations and maintain open communication with pharmacists regarding patient care.
• Deliver exceptional customer experiences that foster trust and high satisfaction among students and their families.
• Promote value-based care initiatives, encouraging health education, preventive measures, and patient adherence to treatment plans.

Patient Population / Client Focus

The primary focus of this role will be on the student population within the school setting, with particular attention to health education and the management of chronic conditions such as asthma, diabetes, and mental health issues. The NP will be a critical resource for low-income families, bridging healthcare access and support, while empowering students to take charge of their health through education and advocacy.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or equivalent Nurse Practitioner program completion.
• Active, unrestricted licensure to practice as a Nurse Practitioner in New York State.
• Active DEA license for prescribing medications.
• Certification from national bodies such as the AANP or ANCC.
• Ability to work collaboratively in a flexible and fast-paced environment.
• Demonstrated commitment to providing compassionate healthcare particularly for senior and underserved populations.

Preferred Experience

• Previous experience in a school or community health setting, particularly with children and adolescents.
• Familiarity with care coordination models and value-based care practices.
• Experience utilizing electronic health record (EHR) systems for documentation and patient management.
