Key Responsibilities\n

• Collaborate closely with primary care providers to identify, address, and manage mental health and psychosocial issues, ensuring a holistic approach to patient care.

• Conduct in-depth patient assessments focusing on diagnostic and functional evaluations, subsequently making informed recommendations to the primary care team regarding treatment goals.

• Deliver targeted therapeutic interventions suitable for patients requiring mental health services, tailoring approaches based on individual needs and circumstances.

• Guide primary care providers in determining patient suitability for ongoing primary care versus referral to specialized mental health services.

• Take initiative with follow-up procedures to assess patient progress and determine necessary adjustments in treatment plans.

• Develop actionable relapse prevention strategies, providing support to patients in maintaining balanced functioning.

• Identify at-risk patients and formulate proactive plans to prevent deterioration of their mental health.
\n
• Coordinate healthcare services for patients in the behavioral health sector, ensuring integrated care with both internal and external resources where appropriate.

• Accurately document patient progress and diagnostic information in medical records while maintaining communication with primary care providers.

• Support patients in managing physical health conditions that may affect their mental well-being, empowering them to make positive lifestyle choices.

• Equip patients with self-management strategies and educational resources to enhance their participation in their own care and recovery journey.

• Offer consultation to clinic management on behavioral health services and participate in identifying metrics for program assessment and outcomes evaluation.

• Assist patients in adhering to medical treatments initiated by primary care, including strategies for coping with medication side effects.

• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals, ensuring all practices meet required ethical standards.

Patient Population / Client Focus\n

This role primarily focuses on serving veterans and their families facing a variety of mental health challenges. The counselor will engage with patients of diverse backgrounds, addressing issues such as anxiety, depression, PTSD, and other psychosocial conditions. A strong emphasis will be placed on creating a safe, welcoming environment where patients feel comfortable expressing their needs and concerns.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling from an accredited institution is required.

• Possession of a current LPC (Licensed Professional Counselor) license.

• Understanding of DSM-IV diagnoses and behavioral therapy techniques.

Compensation and Benefits\n

This position offers a competitive salary range of USD 87,000 to 105,000 per year, commensurate with experience and qualifications. Additionally, we provide a sign-on bonus for selected candidates as an incentive to join our team.
