Position Overview

Sun Valley Heart and Vascular is seeking a dedicated full-time Nurse Practitioner specializing in Cardiology to join our dynamic team located in Valley Stream, New York. In this role, you will be integral to the outpatient practice of the Center for Advanced Cardiac Care (CACC), collaborating closely with an experienced team of cardiologists, nurse practitioners, and other healthcare professionals to provide high-quality care to patients with various cardiovascular issues.

Work Environment

This position is based onsite at a school setting, where you will have the opportunity to engage with a unique patient population, focusing on educational health interventions related to cardiovascular wellness. The typical work schedule is Monday through Friday, from 9 am to 5 pm, without the expectation of on-call duties, night shifts, or weekends, which allows for a healthy work-life balance.

Key Responsibilities

• Collaborate with a team of cardiologists to evaluate and manage patients with hypertension, high cholesterol, and various acute cardiac conditions.
• Conduct thorough patient assessments by performing detailed histories and physical examinations, and determine appropriate treatment plans.
• Prescribe medications and therapies as deemed necessary, ensuring effective management of cardiovascular diseases including coronary artery disease, congestive heart failure, and peripheral artery disease.
• Utilize outpatient-only electrophysiology services to assist patients with abnormal electrocardiograms and arrhythmias.
• Execute stress testing procedures, interpreting results and providing insights for both patients and physicians.
• Provide comprehensive education to both patients and family members on cardiovascular health, lifestyle modifications, and the management of chronic conditions.
• Refer patients to specialists within the organization as required based on individual health needs and treatment protocols.
• Participate in continuous education and training to stay abreast of innovations in cardiology care.

Patient Population / Client Focus

The Nurse Practitioner in this role will primarily engage with an outpatient population that may vary in age and background but typically includes individuals with a focus on cardiovascular health. You will play a crucial role in addressing not just acute presentations but also the long-term management and prevention of heart disease. The insights and care you provide directly contribute to improving the overall health and productivity of our community.

Qualifications

• Completion of a Master’s or Doctoral degree from an accredited Nurse Practitioner program, with a focus on Family or Adult-Geriatric healthcare.
• Current Nurse Practitioner certification and state licensure to practice in New York.
• Demonstrated ability to pass relevant certification exams to ensure high standards of care within the cardiology specialization.
• Strong interpersonal skills and the ability to work collaboratively in a team environment.

Preferred Experience

Experience in an outpatient cardiology setting is highly desirable, but we welcome applications from both seasoned Nurse Practitioners and newer graduates with a commitment to advancing their skills in cardiac care. Prior experience in managing chronic illnesses and familiarity with electrophysiological interventions will be an asset.

Compensation and Benefits
