You will be using an advanced Electronic Medical Record (EMR) system to manage patient interactions, access treatment plans, and document progress notes. Key Responsibilities\n

• Perform initial and ongoing assessments to identify and meet the mental health needs of patients, adjusting treatment plans as necessary.

• Foster effective communication with supervisors, family members, and collateral contacts regarding patient progress and treatment modifications.

• Develop a detailed schedule that includes appointments, home visits, and monitoring observations to ensure patient progress is tracked.

• Coordinate home visits that comply with in-home observation requirements, including conducting chart reviews and staff interviews.
\n
• Oversee the effective implementation of program plans and ensure adherence to therapeutic protocols.
\n
• Actively participate in Interdisciplinary Team Meetings, contributing insights and collaborative strategies for holistic care.
\n
• Document comprehensive monthly and quarterly progress notes reflecting patient advancements and needed adjustments to care.
\n
• Organize training sessions for staff members to enhance competencies regarding the program plan and necessary documentation.
\n
• Conduct follow-up sessions through interviews, home visits, and observations to provide ongoing support and reinforce treatment goals.

• Maintain meticulous records for swift access by relevant parties while ensuring compliance with legal and ethical standards.

• Serve as a liaison with the broader community and mental health organizations to support resource development and community health initiatives.

Patient Population / Client Focus\n

Our counselors work with a diverse clientele, including adults experiencing anxiety, depression, trauma, and substance misuse, as well as families in need of couples or family therapy. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Possession of an active and unrestricted license as a Professional Counselor in Texas.

• A minimum of two years of counseling experience, particularly within a clinical context, is essential.

• Demonstrable expertise in family therapy, couples therapy, substance misuse, or trauma recovery is highly preferred.

• A robust understanding of legal, ethical, and regulatory standards in the mental health field.
