Canyon Physical Therapy Center is seeking a dedicated and skilled Physical Therapist (PT) to join our team in Fort Lauderdale, Florida. This is a full-time, onsite position focused on providing exceptional care to patients within the comfort of their own homes. Work Environment

As a home care physical therapist, you will work independently while benefiting from a strong support network of interdisciplinary healthcare professionals. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians, systematically assessing their physical condition and needs.

• Develop individualized treatment plans grounded in research and tailored to each patient's specific conditions, especially focusing on orthopedic specialties.

• Implement therapy sessions in patients' homes using advanced treatment techniques, ensuring alignment with each patient's rehabilitation goals as outlined in the physician's plan of care.

• Deliver exceptional customer service, actively engaging with patients to foster a welcoming and motivating environment.

• Collaborate with a multidisciplinary team to optimize treatment outcomes through evidence-based practices and continuous patient assessment.

• Educate patients and their caregivers about treatment protocols, empowering them with knowledge to support their recovery process.

• Meet minimum utilization goals and business objectives while prioritizing the overall effectiveness of treatment.

Patient Population / Client Focus

Your primary focus will be working with a diverse patient demographic, predominantly orthopedic patients recovering from injuries, surgery, or chronic conditions. You will be instrumental in providing care tailored to meet the unique needs of each patient, ensuring that you help them regain their mobility and independence in a setting that is both comfortable and familiar.

Qualifications

• Graduation from an accredited physical therapy program approved by the APTA or CAPTE.

• Possession of a Doctorate degree in Physical Therapy and a valid state license to practice.

• Current Basic Life Support certification from the American Heart Association.

• Exceptional verbal and written communication skills, enabling you to effectively interact with patients and their families.

• Proficiency in word processing and computer skills required for documentation and patient care.

• A strong commitment to building trust and relationships with patients to exceed their functional goals.

• A positive disposition and the ability to connect genuinely with individuals.

• Experience building and maintaining community relationships to enhance patient care and awareness.

Preferred Experience

While specific years of experience are not required, candidates with prior home care or outpatient physical therapy experience dealing with orthopedic patients are strongly preferred. Proficiency in evidence-based therapeutic techniques will set you apart as a competitive candidate.
