Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing their medical history and performing the necessary assessments to develop customized treatment plans.
• Implement treatment protocols that are responsive to the patient's evaluation findings with an emphasis on enhancing functional abilities and promoting independence.
• Maintain meticulous documentation of patient interactions, treatment plans, progress notes, and discharge summaries, ensuring compliance with the evolving healthcare regulations.
• Consult with fellow healthcare professionals, addressing changes in patient conditions that may require adjustments to the treatment plan or further medical intervention.
• Participate in facility meetings and training sessions to stay current with best practices and to ensure adherence to state regulations regarding physical therapy.
• Advocate for patient needs, ensuring that a clear understanding of their therapy requirements is communicated and documented effectively.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program is required.
• A valid state license as a Physical Therapist is mandatory, obtained from the Physical Therapy Examining Committee.

• A current CPR certification is necessary to ensure patient safety.

Preferred Experience

While a minimum of two years of relevant experience is required, preference will be given to candidates who have worked in home care settings or have experience with a variety of treatment modalities. Compensation and Benefits
