The usual work hours are from Monday to Friday, 8:00 AM to 4:30 PM, with occasional on-call duties and the potential for evening or weekend hours based on client requirements. Candidates must possess flexibility in their scheduling to provide consistent support and address urgent client needs effectively.

Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments for seniors to evaluate their emotional, mental, and social well-being.

• Collaborating with clients and their families to create personalized care plans that respect individual goals, preferences, and needs.

• Providing counseling and emotional support for seniors and their families during life transitions, helping them navigate challenges and complexities.

• Advocating for clients' rights and facilitating their access to community resources, ensuring they receive necessary support and services.

• Working with interdisciplinary teams composed of nurses, caregivers, and healthcare professionals to ensure seamless care coordination.

• Conducting regular client check-ins to monitor their overall well-being, assess living conditions, and promptly address any health or safety concerns.

• Facilitating effective communication among family members involved in client care, addressing conflicts and fostering collaboration.

• Educating clients and their families about available community resources, support networks, and engaging programs designed to enhance social interactions and quality of life.

• Maintaining accurate and timely documentation of client assessments, care plans, progress notes, and all relevant interactions in compliance with organizational standards.

You will primarily work with seniors facing terminal illnesses and their families, helping them cope with emotional stressors, ensuring their dignity, and enhancing their quality of life during challenging times. Your focus will be on providing holistic support that considers the unique needs and experiences of elderly clients.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Minimum of two years of relevant experience in social work, particularly in community settings or hospice care.

• In-depth knowledge of social work principles, aging issues, and available community and healthcare resources.

Preferred Experience\n

While a minimum of two years of experience is required, candidates with a robust background in geriatric social work or hospice environments will be viewed favorably. Familiarity with community resources in the Bakersfield area is also advantageous.

Compensation and Benefits\n
