Full-Time Orthopedic Physical Therapist for Home Care

Work Environment

This position involves onsite work primarily in a home care settings, enabling you to engage directly with patients in the comfort of their own environments. You will work Monday through Friday from 9 AM to 5 PM, allowing for a structured yet rewarding schedule. Your daily routines will include traveling to various patient locations throughout Trenton and surrounding areas, delivering care that is both compassionate and effective.

Key Responsibilities

• Assess, recognize, and address the specific therapy goals and requirements of each patient, conducting thorough evaluations to inform decisions.
• Maintain a manageable patient load, seeing one patient per hour, ensuring quality interaction and comprehensive care.
• Utilize your expertise in orthopedic outpatient settings to implement therapeutic interventions aimed at improving mobility and functional independence.
• Administer effective treatment modalities and therapies that prioritize enhancing range of motion and decreasing reliance on medications.
• Design and implement comprehensive treatment plans based on a variety of therapeutic techniques, catering to the diverse physiological needs of patients.
• Develop customized fitness and wellness programs that empower patients to engage actively in their rehabilitation.
• Document all services provided accurately and maintain open lines of communication with patients regarding their progress and treatments.
• Collaborate with a multidisciplinary team of healthcare providers, including physicians and occupational therapists, to ensure a holistic approach to patient care.
• Evaluate treatment outcomes, adapting therapeutic techniques accordingly, to achieve optimal results.

Patient Population / Client Focus

You will work with a diverse population of patients, predominantly in need of orthopedic rehabilitation. Your efforts will focus on individuals recovering from surgeries, injuries, and chronic conditions impacting mobility. Through personalized care, you will help them regain strength, flexibility, and functional independence.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program is required.
• Possession of an active and valid Physical Therapist license in New Jersey.
• History free of any board or disciplinary actions on your license.
• Proficient in manual therapy techniques, exercise program design, and implementing evidence-based practice.
• Familiarity with electronic medical record (EMR) systems, particularly WebPT, for documentation and patient tracking.

Preferred Experience

• Previous experience working as a Physical Therapist, particularly in orthopedic and outpatient care settings, is highly preferred.
• Ability to effectively engage with patients to motivate them throughout their rehabilitation journey.
• A strong foundation in evidence-based therapeutic practices that enhance patient outcomes.

Compensation and Benefits
