Physical Therapist - Full Time - Home Health Care

Create customized treatment plans focused on preventing disabilities, relieving pain, and restoring functionality, while closely adhering to physician recommendations.

Collaborate with physical therapy assistants to ensure the correct implementation of care plans, and provide guidance and supervision in their responsibilities.

Monitor and document patient progress, responding to changes in their condition and adjusting treatment approaches as needed.

Participate actively in interdisciplinary team meetings, contributing insights from rehabilitation perspectives to enhance overall care strategies.

Engage with patients' families to educate and support them in understanding and implementing care protocols at home.

Maintain accurate clinical records, ensuring timely and compliant submission of evaluations, progress notes, and discharge summaries.

Stay informed about the latest research and best practices in orthopedic rehabilitation to continually enhance the quality of care provided.

You will primarily work with patients recovering from surgeries, sports injuries, and various musculoskeletal conditions. Our patients vary in age and background, ranging from young athletes to older adults, and each presents unique challenges and opportunities for rehabilitation. Your ability to connect with and motivate patients will be essential in fostering a trusting therapeutic relationship, allowing you to tailor interventions that meet their specific needs.

Bachelor's Degree in Physical Therapy from an accredited institution.

Active and unrestricted Physical Therapy licensure in New Jersey.

A minimum of one year of clinical experience in physical therapy, preferably with a focus on orthopedic rehabilitation.

Current CPR certification to ensure the safety of all patients during therapy sessions.

Valid driver's license and automobile insurance for transporting oneself to patient homes or community facilities if required.

A commitment to ongoing professional development and a passion for delivering high-quality care.

Experience working in a rehabilitation center or similar healthcare environment.

Familiarity with electronic health records (EHR) systems to manage documentation efficiently.

Demonstrated ability to mentor and guide junior staff or Physical Therapy Assistants in best practices.
