Bedford Healthcare Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity. This full-time position offers you the opportunity to make a significant difference in the lives of patients and their families by providing critical support and mental health services in a hospice center environment. You will work Monday to Friday from 9:00 AM to 5:00 PM, contributing to the well-being of individuals facing serious health challenges.

Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments to evaluate mental health, substance use, and medical needs in a hospice setting, ensuring care is age and culturally appropriate.

• Provide comprehensive behavioral health services, including individual and group counseling, crisis intervention, treatment planning, and creating referrals for substance abuse treatment as necessary.
\n
• Perform and document mental health evaluations and progress notes electronically, ensuring that each patient’s journey is accurately recorded and accessible to the care team.
\n
• Actively participate in ensuring that patient needs are escalated to appropriate medical providers in a timely manner, enhancing overall care coordination.
\n
• Collaborate with quality improvement teams to develop and implement processes for collecting and analyzing clinical data, improving service delivery and patient outcomes.
\n
• Facilitate connections to local resources and community agencies for additional support, including medication assistance programs and other services beneficial to patients and families.

Patient Population / Client Focus\n

In this role, you will serve a diverse patient population within a hospice care environment. Patients will often be dealing with complex emotional responses to life-limiting illnesses, and you will have the opportunity to support them and their families through counseling and guidance, focusing on enhancing quality of life during their remaining time.

Qualifications\n

• A valid and current LCSW license to practice in the state of California.
\n
• A Master’s degree in social work (MSW) with at least three years of experience post-licensure in a clinical setting, preferably in hospice or palliative care.

• Demonstrated expertise in counseling techniques, crisis intervention strategies, and the ability to exercise independent judgment regarding direct services to clients.

• Strong experience in group facilitation, planning processes, and working collaboratively within multidisciplinary teams.

Preferred Experience\n

• Experience working specifically within hospice care or with terminally ill patients.

• Familiarity with electronic health record (EHR) systems and virtual healthcare delivery methods.

• Proven ability to create effective treatment plans and conduct culturally sensitive assessments for diverse populations.

Compensation and Benefits\n
