Work Environment

This role is onsite, primarily conducted in the homes of our clients across Front Royal and surrounding areas. Key Responsibilities

As a Licensed Home Health Physical Therapist, your daily tasks will include:

• Directing physical therapy treatment plans aimed at preventing disabilities, relieving pain, and restoring function while adhering to individual care plans.
• Assessing and evaluating clients' functional levels through thorough diagnostic tests, identifying areas of improvement and necessary interventions.
• Coordinating with and supervising Physical Therapy Assistants to ensure the accurate execution of therapy plans.
• Creating and adjusting therapy plans based on the client's needs, progress, and environment, offering solutions to improve their independence.
• Delivering hands-on therapeutic exercises, engaging clients in activities that support their recovery, including both manual and non-manual techniques.
• Monitoring clients' conditions and modifying treatment approaches as necessary while documenting all clinical records comprehensively.
• Participating in interdisciplinary team discussions for holistic patient care, providing insights for improving treatment outcomes.
• Conducting timely assessments, including initial evaluations, progress notes, and discharge summaries to support ongoing care strategies.

Patient Population / Client Focus

Your primary focus will be on patients requiring home health services, including those recovering from surgery, managing chronic conditions, or seeking rehabilitation after injury. Our patient demographic includes older adults, individuals with disabilities, and those in need of palliative care. By working closely with patients and their families, you'll tailor therapy approaches to meet unique challenges, promoting a higher standard of care.

Qualifications

The ideal candidate will possess:

• A Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy is preferred.
• Current and valid state licensure as a Physical Therapist, maintaining good standing.
• Certification in CPR, first-aid, and basic life support applicable to a home care setting.
• A valid driver's license and reliable transportation, with automobile insurance.
• Strong interpersonal skills with a passion for patient engagement and advocacy.

Preferred Experience

A minimum of one year of previous experience in a physical therapy role, particularly in home health or acute care settings, is highly desirable. Familiarity with community resources and regulations governing home healthcare can be beneficial. A proven ability to make informed clinical decisions promptly will be a significant asset in this role.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 100,000 to 120,000, accompanied by a comprehensive benefits package.
