Work Environment

This role is primarily onsite, requiring travel to patients' homes in and around the Jacksonville area. Our home care approach means you'll have the unique opportunity to see patients in their own environment, facilitating a personalized treatment experience. You will work Monday to Friday, maintaining a flexible schedule to accommodate patient needs and ensuring optimal therapy sessions that align with their routines and lifestyles.

Key Responsibilities

• Conduct thorough evaluations of patients, which include reviewing medical histories, performing physical assessments, and documenting findings to inform tailored treatment plans.
• Communicate treatment plans effectively to patients and their families, involving them in the goal-setting process for enhanced engagement and adherence.
• Implement specialized treatment protocols based on evaluation results while adjusting methods as needed to meet the shifting health care landscape.
• Collaborate with multidisciplinary healthcare teams to ensure comprehensive patient care, addressing any necessary modifications based on patients' changing conditions.
• Oversee discharge planning and follow-up care, ensuring proper documentation and billing for all patient interactions, which captures the full scope of their care journey.
• Manage your schedule effectively to meet productivity goals, demonstrating punctuality and reliability throughout your involvement with meetings and patient care.
• Contribute to department objectives through participation in quality improvement initiatives and compliance with relevant regulations.
• Actively pursue continuing education opportunities to enhance your professional skills while influencing the growth of the organization.
• Facilitate patient access by participating in outreach initiatives and removing barriers to care, thus improving overall patient experience.

Patient Population / Client Focus

As a Physical Therapist at Pathways, you will primarily work with adults and seniors recovering from surgeries, injuries, or managing chronic conditions. This demographic may include patients experiencing mobility issues, pain management needs, and rehabilitation requirements after orthopedic procedures. Your skills will greatly affect their recovery outcome and overall quality of life.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• A current physical therapy license as required in the State of Florida.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Strong interpersonal and communication skills that facilitate effective patient interactions and teamwork.
• Ability to adapt treatment plans to meet the diverse needs of patients and situations.

Preferred Experience

• A minimum of two years of relevant hands-on experience in a physical therapy setting.
• Previous experience in home health care or rehabilitative services is strongly preferred, but not mandatory.

Compensation and Benefits
