Work Environment

This position is primarily onsite, with a focus on home care environments. The selected Physical Therapist will conduct assessments and treatment sessions in patients' homes, which allows for personalized care based on the unique dynamics of each situation. Key Responsibilities

• Conduct comprehensive evaluations of patients with various functional limitations and disabilities, developing tailored treatment plans aimed at promoting recovery and enhancing mobility.
• Collaborate effectively with interdisciplinary treatment teams, ensuring open lines of communication with physicians and other healthcare providers about patient progress and treatment outcomes.
• Incorporate evidence-based practices in your treatment approach, continually reassessing and adjusting plans based on patient responses.
• Educate patients and families about conditions and treatment strategies, fostering an understanding of their roles in the recovery process.
• Document treatment plans and patient progress meticulously, adhering to all applicable laws, regulations, and professional standards.
• Participate in community outreach and educational programs, contributing to departmental goals of awareness and engagement.
• Supervise and mentor support staff, ensuring that patient care standards are maintained throughout the treatment process.

Patient Population / Client Focus

Our patient population is diverse, including individuals recovering from surgical procedures, chronic pain management needs, and rehabilitation following trauma or neurological events. The ideal candidate will exhibit a strong cultural competence to effectively interact with patients from various backgrounds, adapting treatment modalities to meet their specific needs.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current state licensure as a Physical Therapist in Florida.
• Exceptional communication skills, both verbal and written, with a keen understanding of documentation standards.
• Basic life support (BLS) certification is mandatory.
• Proficiency in standard office software and electronic health record (EHR) systems is essential for effective documentation and billing processes.

Preferred Experience

While two years of clinical experience is required, candidates with backgrounds in home health care or rehabilitative services will be especially valued. Experience working with diverse patient populations and interdisciplinary teams is a plus.

Compensation and Benefits
