Key Responsibilities\n

• Conduct thorough assessments to evaluate clients' social, emotional, and environmental challenges, particularly in the context of end-of-life care.

• Develop comprehensive service plans tailored to each patient’s needs, including actionable goals, interventions, and progress tracking.

• Deliver compassionate counseling tailored for individuals and families dealing with grief, anticipatory loss, and adjustment to terminal illness.

• Guide patients and clients in creating personalized plans, ensuring they have access to vital community resources and support services.

• Offer immediate crisis intervention and risk assessment to address any psychological emergencies as they arise.

• Foster collaborations with multidisciplinary teams and external agencies to enhance service coordination and advocacy for our patients.

• Engage in proactive community outreach initiatives to increase awareness of hospice services and resources available in Lakewood and surrounding areas.

• Regularly participate in team meetings, case conferences, and ongoing training to stay updated on best practices and continuing education opportunities.

Patient Population / Client Focus\n

The primary focus of this role is on patients facing terminal illnesses and their families. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
