The role is based onsite at our state-of-the-art rehabilitation center in Centreville. Key Responsibilities

• Conduct thorough evaluations of patients by collecting pertinent medical histories and performing specialized assessments to determine treatment needs and goals.
• Engage in shared decision-making with patients and their families, clearly explaining evaluation findings and collaboratively developing tailored treatment plans that align with patient goals.
• Implement treatment protocols based on evidence, adjusting methods as necessary to accommodate patients' individual responses and changes in their conditions.
• Consult with interdisciplinary teams, recognizing and documenting any required modifications to treatment plans based on ongoing assessments and changes in patient health statuses.
• Ensure comprehensive discharge planning and ongoing patient follow-up care, maintaining accurate and timely documentation throughout the treatment process.
• Manage your schedule efficiently to meet productivity expectations while maintaining punctuality for meetings and participation in hospital and community marketing initiatives.
• Contribute to departmental goals, adhering to all regulatory requirements, and actively participating in professional development activities such as workshops and training.
• Enhance customer experiences by delivering compassionate care, while also working with teams to eliminate barriers to patient access.

Patient Population / Client Focus

Qualifications

• Doctorate Degree in Physical Therapy (DPT) from an accredited institution.
