Key Responsibilities\n

• Conduct thorough and ongoing assessments to determine the specific needs of each patient, adapting treatment plans accordingly.

• Maintain consistent communication with supervisors, caregivers, and relevant parties to keep them updated on patient progress and treatment efficiency.

• Prepare a systematic schedule of appointments and home visits, utilizing effective time management to monitor patient progress and enhance service delivery.

• Arrange and conduct home visits as necessary, enabling comprehensive chart reviews and data analysis that inform treatment strategies.

• Ensure all elements of treatment plans are implemented accurately and effectively to contribute to successful outcomes.

• Participate actively in interdisciplinary team meetings, sharing insights and collaborating with professionals across disciplines.

• Document monthly and quarterly progress notes meticulously, ensuring compliance with regulations and facilitating ongoing patient evaluation.

• Organize training sessions for staff to ensure competency in program implementation and documentation, fostering a knowledgeable team environment.

• Provide follow-up support through interviews and home visits to ensure continuous care and adjustment of strategies as needed.

• Act as a vital liaison between the clinic and the broader mental health community, fostering partnerships that enhance service delivery and outreach.

Qualifications\n
