Key Responsibilities\n

• Conduct comprehensive assessments to tailor treatment plans that address individual client needs.

• Maintain clear and effective communication with supervisors, care staff, and families regarding client progress and treatment outcomes.

• Develop and share scheduled appointments and necessary follow-up home visits to monitor ongoing client engagement.

• Coordinate necessary home visits to perform in-depth observations, engage in chart reviews, and analyze relevant data.

• Ensure that all aspects of treatment plans are executed effectively and adjust as necessary based on client feedback.

• Participate actively in interdisciplinary team meetings and other relevant gatherings to discuss patient care strategies.
\n
• Document monthly and quarterly progress notes to effectively track client development and treatment efficacy.
\n
• Provide training sessions aimed at certifying staff on program implementation and documentation processes.
\n
• Conduct follow-up interviews and home visits to reinforce ongoing support for clients.
\n
• Act as a liaison between clients and the broader mental health community, advocating for their needs and supporting development goals.

• Assist in creating and refining training materials that aim to enhance service delivery within our programs.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including both adults and youth. You will have the opportunity to engage with individuals facing a variety of mental health challenges, such as anxiety, depression, trauma, and substance abuse. Your ability to tailor your approach to meet the unique needs of each client is critical, as you help them navigate their mental health journeys.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Illinois.

• A minimum of two years of professional counseling experience, preferably within a clinical setting.

• Competence in family therapy, couples therapy, substance misuse treatment, or trauma-informed care is strongly preferred.

• A deep understanding of legal and ethical standards within the mental health field.

• Strong interpersonal and communication skills, capable of establishing rapport with various clientele.

• Proven ability to collaborate effectively within a mult-disciplinary healthcare team.

• Familiarity with electronic medical records (EMR) software and related technological tools.
