Key Responsibilities\n

• Conduct ongoing assessments to determine and address the evolving needs of each patient, ensuring tailored care plans are beneficial and effective.

• Maintain open lines of communication with supervisors, direct care staff, families, and guardians regarding patient progress, adapting treatment strategies as necessary.

• Regularly provide a detailed schedule of appointments and follow-ups to management to ensure continuity of care and successful monitoring of program implementation.

• Coordinate home visits as needed for comprehensive assessment, which may include chart reviews, staff interviews, and detailed data analysis relevant to patient care.

• Ensure that all therapeutic interventions align with established program plans and that they are delivered in accordance with organizational standards.

• Engage in interdisciplinary team meetings and additional meetings to discuss case developments and refine patient care strategies.

• Prepare and submit monthly and quarterly progress notes that accurately reflect the patient’s journey and treatment efficacy.

• Organize and conduct training sessions for staff to ensure competency in program implementation and adherence to documentation standards.

• Provide ongoing follow-up through interviews, home visits, and observations to support the journey and achievements of each individual.

• Work as a liaison with the mental health community to establish and support the organization’s developmental goals, enhancing community integration.

• Develop procedures for training materials aimed at optimizing programs designed for individual client improvement.

As a Licensed Clinical Professional Counselor at Riverwoods Academy, you will work with a diverse range of clients, including both adults and youth. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as an LCPC in the state of Illinois.
