Key Responsibilities\n

• Conduct thorough assessments to identify and address the needs of patients, ensuring that personalized care plans are tailored to each individual.

• Facilitate clear communication between supervisors, direct care staff, family members, and guardians about patients' progress, treatment plans, and needs.

• Develop and share a tentative schedule with supervisors regarding appointments, home visits, and monitoring activities to ensure consistent support for patients.

• Coordinate home visits, ensuring compliance with requirements for data gathering through chart reviews and staff interviews, facilitating comprehensive observations for continuous support.
\n
• Ensure that program plans are effectively implemented, monitored, and adjusted as necessary to meet patients' evolving needs.
\n
• Participate actively in interdisciplinary team meetings and relevant discussions to enhance collaborative care strategies.
\n
• Compile and document monthly and quarterly progress notes to maintain accurate records of patient progress.
\n
• Lead training initiatives to ensure that all team members are well-prepared to implement program plans and maintain accurate documentation.

• Provide ongoing follow-up through interviews, home visits, and observations aimed at continuous patient support.
\n
• Maintain organized records for easy access and review by authorized parties, ensuring compliance with legal and ethical standards.

• Act as a liaison with the broader mental health community, fostering partnerships that support program development and community outreach goals.

• Assist in creating training materials and procedures to enhance staff competencies and improve intervention strategies for individuals.

Patient Population / Client Focus\n

The Licensed Clinical Professional Counselor will work with a diverse patient population, including both adults and youth. The focus will be on addressing a wide range of mental health issues, including but not limited to anxiety, depression, trauma, substance misuse, and family dynamics. Your expertise in family therapy, couples therapy, and trauma-informed care will be invaluable as you provide support and guidance to those in need.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted licensure as a Licensed Clinical Professional Counselor (LCPC) in the state of Maryland is essential.
