Key Responsibilities\n

• Conduct thorough, ongoing assessments to determine the specific needs of each patient, adjusting strategies as necessary to ensure optimal care.

• Facilitate clear and professional communication among all stakeholders, including supervisors, direct care staff, family members, and guardians, to discuss individual progress and treatment plans.

• Develop and propose a tentative schedule for client appointments, home visits, and follow-up observations that align with organizational goals for monitoring advancement.

• Plan and coordinate home visits in accordance with in-home observations, chart reviews, and data analysis for comprehensive assessments.

• Ensure that the implementation of program plans is executed effectively, monitoring for compliance and adjusting as needed.

• Participate actively in interdisciplinary team meetings, bringing insights and recommendations to support collaborative care for patients.

• Timely completion of monthly and quarterly progress notes to document patient development and treatment adjustments.

• Organize and provide training for staff and team members to enhance implementation of program plans and accurate documentation.

• Conduct follow-up interviews and observations to ensure sustained support and attention to evolving individual needs.

• Maintain an organized record-keeping system to ensure pertinent information is easily accessible for all interested parties.

• Act as a liaison between the clinic and the broader mental health community to bolster development initiatives.

Patient Population / Client Focus\n

Qualifications\n

• Active and unrestricted licensure as a Licensed Clinical Professional Counselor (LCPC) in Illinois is required.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is essential.

• Minimum of 2 years of clinical experience in counseling or a related field, demonstrating competency in assessment and therapy.
