Key Responsibilities\n

• Conduct thorough, ongoing assessments of patient needs to tailor appropriate therapeutic interventions.

• Serve as a vital communication link between supervisors, direct-care staff, family members, and other stakeholders regarding the progress of individual cases.

• Develop and provide tentative schedules for appointments, home visits, and necessary observations to ensure effective monitoring of patient care.
\n
• Coordinate and carry out home visits, which may include performing in-home observations, reviewing patient charts, and conducting interviews with family and staff.
\n
• Ensure adherence to the outlined program plan and reinforce the implementation of therapeutic strategies for optimal patient outcomes.
\n
• Collaborate actively in Interdisciplinary Team Meetings, contributing insights and updates on patient progress when required.
\n
• Maintain detailed records and documentation, ensuring all relevant data is accessible to authorized parties to support clinical best practices.

• Organize and lead training sessions aimed at certifying staff members on the program plan and related documentation processes.
\n
• Provide ongoing support through follow-up interviews and observations to adjust care according to the evolving needs of patients.

• Act as an ambassador within the mental health community, actively engaging in discussions that further public understanding and resources for mental health services.

Patient Population / Client Focus\n

Your role will involve working with a varied clientele, including both adults and youth, who may present with a diverse range of mental health challenges. You will be integral in providing support for issues such as anxiety, depression, trauma, familial conflicts, and substance misuse. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted license as a Professional Counselor in the state of Arizona.

Preferred Experience\n
